Who Qualifies for an Oral GLP-1 Prescription in 2026

BMI thresholds, comorbidity requirements, insurance criteria — here's who can actually get prescribed oral semaglutide and through which channels.

FDA-approved indications

The Wegovy pill (25mg oral semaglutide) is FDA-approved for chronic weight management in adults with a BMI of 30 or greater (obesity), or a BMI of 27 or greater (overweight) with at least one weight-related comorbidity such as hypertension, Type 2 diabetes, or dyslipidemia. Rybelsus is approved for Type 2 diabetes specifically, not for weight management.

Insurance requirements vs. clinical eligibility

Clinical eligibility and insurance coverage are different things. Your prescriber may determine that you are clinically appropriate for oral semaglutide. Your insurance company may still deny coverage based on their own criteria, which often include more restrictive BMI thresholds, documentation of failed prior weight loss attempts, or specific comorbidity requirements.

Prior authorization is almost universal for GLP-1 medications. Expect your prescriber to submit documentation including your BMI, relevant lab work, comorbidity diagnoses, and often evidence of previous weight loss interventions.

Contraindications

Oral semaglutide should not be used by patients with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2 (MEN2), patients with a history of pancreatitis, or pregnant or breastfeeding patients. A thorough screening for these contraindications is a baseline clinical requirement — any prescriber who skips this screening is not meeting standard of care.

Telehealth access

Several telehealth platforms now prescribe oral semaglutide for weight management. Sesame Care prescribes FDA-approved brand-name medications including the Wegovy pill. What to bring to your consultation

Whether in-person or virtual, come prepared with your current weight and height (for BMI calculation), a list of current medications, relevant medical history (especially GI conditions, thyroid conditions, and pancreatitis history), recent lab work if available (metabolic panel, A1C), and your insurance information for formulary checking.
